Backflow Device Report

Use this form to lodge a test report and make a payment to the Environment and Planning Directorate in relation to the continued operation and testing of a backflow device.

Where a testable backflow prevention device is installed, the Regulations contain some very specific requirements for testing and reporting. These will also have to be met before booking a final inspection. These devices are also required to be tested annually to ensure that they continue to operate as required and a test report submitted to Environment and Planning Directorate. Only plumbers who have successfully completed a backflow prevention course are permitted to commission and test these devices.

Important Notice: When completing an annual form, please refer to the owner's Notice of Inspection issued by Environment and Planning Directorate to the owner of the Backflow Device.
